Under 18 forms

All international students who will be under 18 years of age must register their proposed accommodation and welfare arrangements with the University by completing an Under 18 form that is relevant to their situation.

Please click an option below that best describes your proposed care arrangements for your stay in Australia to download the relevant form:

Option 1:
I will stay with (one of) my parents/legal custodian
Download Under 18 Form - Option 1
Option 2:
I will stay with an approved* relative
Download Under 18 Form - Option 2
Option 3a:
I will stay at a University of Melbourne Residential College
Download Under 18 Form - Option 3a
Option 3b:
I will have my accommodation arrangements assessed by the University and enrol in the Under 18 Supervision Program

*A relative can be your (step)brother, (step)sister, (step)grandparent, (step)aunt, (step) uncle, (step)niece or (step)nephew. He or she must also be over 21 years of age and of good character
